Services We Provide

      

Service Coordination
Service Coordinators work with individuals and their families to explore their dreams and goals, and to access resources and services available to all citizens, as well as specialized services and supports available through the agency.  They facilitate and document the agreements between the individual receiving supports and those providing them, and monitor service delivery and individual satisfaction with supports and services.

Community Support Services
The Community Support Services is an independent living program for people
who are able to live independently or with minimal assistance.  People are referred to the services by their service coordinator.

Family Support Services
Five Family Support staff assist families who have a child or adult with developmental disabilities living at home.  Services include respite, school to adult transition support, recreation and leisure activities, parent to parent linkages, resource information, and other kinds of individualized help.
