Intake & Eligibility
The CDS Intake Coordinator will provide further clarification and will help you through the intake process if you decide to apply for services for yourself or someone you know. There are Eligibility criteria in three different categories:
Family-Centered Early Supports
and Services:
Provided in natural settings for
families residing in New Hampshire with children, birth until the child's
third birthday, who have developmental disabilities, are at risk for substantial
developmental delays, or have established conditions (He-M 510.01).
