Family-Centered Early Supports and Services
Intake Process

1.  A Parent, Pediatrician, Family Doctor, Hospital or Other Person contacts the
     CDS Intake Coordinator to make a referral (ESS system has 45 days to
     complete process from date of referral)

2.  The Intake Coordinator schedules intake with Family.

3.  The Intake Coordinator completes Intake with Family.  (Eligibility
     criteria/categories shared, family’s rights reviewed, medical and developmental
     history obtained, preliminary developmental information gathered from parent
     report on Ages and Stages Questionnaire, ESS program choices described as
     well as community resources, permissions and releases signed).

4.  Family chooses an ESS Program for eligibility evaluation and notifies the Intake
    Coordinator of their choice. (At the Intake or Later).

5.  Intake Coordinator makes referral to the ESS Program family has chosen.

6.  ESS Program schedules eligibility evaluation. (A play-based assessment with a
     minimum of two qualified examiners to be done in a child’s home or other
     natural environment).

 Family chooses program for services            Family may appeal/request review

 IFSP written within 45 days of referral         Next Ages & Stages form offered

 Developmental services begin                      May be referred again to ESS

 Review IFSP in 6 months or less                 Community resources identified

 Yearly developmental assessment                “Child Check” information shared
 
 

ESS ENDS AT AGE THREE

Transition to community resources, possibly including school district special education.  Ongoing eligibility for Area Agency services reviewed (child must have or appear to have a developmental disability).
