Supports & Services to Individuals with Developmental Disabilities & Brain Injuries
Intake Process

1.  An Individual, Parent, Pediatrician, Family Doctor, Hospital or Other Person
     Contacts the CDS Intake Coordinator to Make a Referral
     (AA system has 21 to 35 days to complete process from date of referral).

2.  The Intake Coordinator schedules intake with Family or Guardian.

3.  The Intake Coordinator Completes Intake with the Individual/Guardian
     (Eligibility criteria and individual’s rights reviewed, medical and developmental
     history obtained, area agency services described as well as community
     resources, application completed, permissions and releases signed).

4.  Coordinator Requests Documentation needed to determine eligibility from
     Sources such as Physician, School District, Medical Specialists, etc.

5.  Eligibility Determination Committee reviews available documentation to
     determine eligibility within 21 or 35 days depending upon timeframe approved
     by applicant/guardian (An ineligible determination must be made within 21/35
     days if sufficient documentation has not been received to support eligibility).

PERIODIC REVIEW OF CONDITIONAL ELIGIBILITY

Conditional eligibility for Area Agency services is reviewed by the Eligibility Determination Committee from time to time.

Prior to age 22 Unconditional Eligibility must be established for ongoing Area Agency services.
